You can breastfeed without a special breastfeeding shirt, but it's much, much easier with one. Every mother who tries out nursing t-shirts just doesn't like to breastfeed in regular clothes that have to expose either the whole chest or the belly. Breastfeeding in public in "non-breastfeeding" clothes also usually attracts unwanted attention.
